27Quick Release Victaulic Pipe Couplings Without Safety Clips Could Be Deadly

Brief Description of Incident/Accident & Description of Consequences/Outcomes:

An operator was knocked over by a blast of compressed air that was inadvertently released from a pipe range. A crew was removing a festoon train from a longwall main gate when it became apparent that it might foul the water range. It was decided to lift the water range using the Eimco bucket. The water range was isolated before lifting commenced. As the water range was raised it caught the locking lever of an over-centre type quick release Victaulic coupling on the compressed air range causing the coupling to open and the compressed air to be released.

Identified Root Causes:

This type of coupling is designed to have a safety clip fitted to the locking lever to prevent it inadvertently opening. The safety clip had not been installed.

The mines investigation concluded that:-

The standard of installation was inadequate with respect to installed height and installation of coupling safety clips

A height survey that was conducted was inadequate

The mine concerned has changed its installation standard to require standard bolt-up type Victaulic couplings to be used in the outbye "panel set-up" section of compressed air ranges that are usually installed by contractors. Quick release type couplings will continue to be installed by the development crews extending the ranges.

Clearly where quick release couplings are used, quality control procedures during installation must be robust, including for example, a verification audit and sign-off.
