9Fatality Involving Mobile Crushing Plant

Brief Description of Incident/Accident & Description of Consequences/Outcomes:

A worker sustained fatal injuries when struck by the right hand hopper door of a mobile crushing plant.

Identified Root Causes:

The worker was in the process of removing the hopper doors so they could be taken off site for repair. All hold down bolts, supporting cross bars and the rear door had been removed. The hydraulic ram used to raise and lower the hopper door was the only thing holding the door in a vertical position. When the pin was dislodged from the hydraulic ram, the hopper door fell uncontrolled.

Preventative/Recommended/Accepted Steps of Risk Mitigation, Points of Interest:

  • All mines must implement a system whereby risk management is used to determine the need for development of written work instructions before commencing a task.
  • All mines must use a risk management process before conducting a task to ensure an acceptable level of risk is achieved. This process can vary from a formal Risk Assessment to the use of a system such as Take Five or Stop Look Assess Management (SLAM).
  • All mines must implement a process that ensures workers have the necessary supervision to achieve an acceptable level of risk.
  • All mines must review their induction process to ensure it complies with all legislative requirements. This should include training in, and utilisation of, risk management techniques.
  • All mines must review their training and assessment processes to ensure they comply with all legislative requirements.
  • Maintenance manuals must be supplied, and the site senior executive must ensure they are available to workers.
