Brief Description of Incident/Accident & Description of Consequences/Outcomes:

While undertaking an inspection at a quarry, a dump truck was found parked up with the tray in the raised position to allow maintenance to be undertaken. A timber chock had been placed on the chassis at the hinge point of the tray. This timber chock had been placed there by the operator to prevent the tray from falling in the event of hydraulic failure. After some discussion it was agreed to demonstrate what effect the weight of the tray would have on the timber chock, simulating a failure of the hydraulics. The timber chock failed catastrophically once the weight of the tray was placed on the timber.

Before any persons place themselves in any area of a truck where the risk of being crushed is identified they must address the potential to cause harm and put in place appropriate engineering controls to protect themselves and other persons. Appropriately designed safety props must be used.
