Information:

Operator was in the process of withdrawing the drill string from a completed hole and had retracted one stem. He had the second stem in the carousel stem lock and commenced rotating the drill head to undo the head from the threaded top of the stem. The retaining lug broke off, fell 13.4m to the deck of the drill and bounced through the window into the operator's cab. It caught the operator a glancing blow on his left ankle and showered him with broken glass from the window. Safety glasses and high leg safety boots prevented further injuries. The drill and a sister machine were both immediately stood down and crack testing carried out. Cracks were found in both stem locking plates.
